Objectives

I.  Correlate gross and histologic morphology of Wilms' tumor with treatment 
and clinical outcome.
II.  Refine treatment according to stage, particularly for the benefit of 
patients requiring minimal therapy.
III.  Test treatment hypotheses.
IV.  Gather data on patients and families to identify those at high risk for 
cancer.
V.  Study the late consequences of successful treatment given for Wilms' tumor.

General Eligibility Criteria:

Previously untreated patients under 
15 years of age with Stages I to IV Wilms' tumor.  Patients must be eligible 
for primary surgical removal of tumor.  As of October 1986, the study is open 
only to patients with Stage II, III, or IV anaplastic tumors; other patients 
with Wilms' tumor should be registered on NWTS-4 (POG-8650).  Children with 
rhabdoid tumors have been dropped from the NWTS.

Expected Enrollment

425 evaluable patients will be required.  As of October 1986, accrual is 
continuing for patients with Stage II, III, or IV anaplastic disease in order 
that results can be better defined statistically; accrual is closed to all 
other patients.

Outline

Randomized study.  All patients undergo surgical excision of tumor, all 
extensions, and involved lymph nodes.  Patients are then randomized according 
to disease stage, as follows:  Stage I, favorable histology (Arm I or II); 
Stage II, favorable histology (Arm III, IV, V, or VI); Stage III, favorable 
histology (Arm IV, V, VII, or VIII); Stage IV, favorable plus all patients 
with unfavorable histology, any stage (Arm IX or X).  As of October 1986, 
patients with anaplastic Wilms' tumors are being randomized to Arms IX and X.
Arm I:  2-Drug Combination Chemotherapy.  Actinomycin-D, ACT-D, NSC-3053; 
Vincristine, VCR, NSC-67574.  10 weeks of therapy.
Arm II:  2-Drug Combination Chemotherapy.  ACT-D; VCR.  As in Arm I, for 6 
months of therapy.
Arm III:  3-Drug Combination Chemotherapy.  ACT-D; VCR (schedule as in Arm I); 
Adriamycin, ADR, NSC-123127.
Arm IV:  3-Drug Combination Chemotherapy plus Radiotherapy.  ACT-D; VCR; ADR; 
plus megavoltage Radiotherapy.
Arm V:  2-Drug Combination Chemotherapy plus Radiotherapy.  ACT-D; VCR; plus 
megavoltage Radiotherapy.
Arm VI:  2-Drug Combination Chemotherapy.  ACT-D; VCR.
Arm VII:  3-Drug Combination Chemotherapy plus Radiotherapy.  ACT-D; VCR; ADR 
(schedule as in Arm III); plus megavoltage Radiotherapy.
Arm VIII:  2-Drug Combination Chemotherapy plus Radiotherapy.  ACT-D; VCR
