Basic Trial Information

Trial Description

Purpose:

Drugs used in chemotherapy, such as docetaxel, oxaliplatin, capecitabine, and fluorouracil, work in different ways to stop the growth of tumor cells, either by killing the cells or by stopping them from dividing. Radiation therapy uses high-energy x-rays to kill tumor cells. Giving more than one drug (combination chemotherapy) together with radiation therapy before surgery may make the tumor smaller and reduce the amount of normal tissue that needs to be removed.

This randomized phase II trial is studying giving docetaxel together with oxaliplatin, capecitabine, fluorouracil, and radiation therapy to see how well it works compared with giving fluorouracil together with oxaliplatin and radiation therapy in treating patients with locally advanced cancer of the esophagus or gastroesophageal junction.

Eligibility:

Eligibility criteria include the following:

• At least 18 years old
• No distant metastasis
• No previous treatment for this cancer

Final eligibility for a clinical trial is determined by the health professionals conducting the trial.

Treatment/Intervention:

Patients will be randomly assigned (have an equal chance of being placed) to one of two treatment groups.

Patients in group one will receive a 1-hour infusion of docetaxel and a 2-hour infusion of oxaliplatin on day 1. They will also receive capecitabine by mouth twice a day for 2 weeks. Treatment may repeat every 3 weeks for up to two courses. Patients will then receive a 5-day continuous infusion of fluorouracil. They will also receive a 2-hour infusion of oxaliplatin in weeks 1, 3, and 5. Patients will also undergo radiation therapy 5 days a week for up to 5½ weeks. Approximately 4-12 weeks later they will undergo surgery.

Patients in group two will receive a 5-day continuous infusion of fluorouracil in week 1. They will receive a 2-hour infusion of oxaliplatin in weeks 1, 3, and 5. Patients will also undergo radiation therapy 5 days a week for up to 5½ weeks. Approximately 4-12 weeks later they will undergo surgery.

Patients will undergo blood sample collection periodically for laboratory studies. Tissue samples will also be taken during biopsy and surgery and studied in the laboratory.

After finishing treatment, patients will be evaluated every 3 months for 2 years.
