Description of Evidence 1

Added text 2 to state that the incidence of breast cancer can be lowered with selective estrogen receptor modifiers, but whether there is a consequent decrease in breast cancer mortality is unknown.

Added text 3 to state that the incidence of breast cancer increased dramatically in the United States and in many European countries between the 1980s and 1990s (cited Holford et al. as reference 39 and Waller et al. as reference 40).
