This clinical trial examines a care transition intervention, CONTINUUM, for hospitalized patients with cancer that has spread from other places in the body (advanced). Patients with cancer often experience challenges, such as symptom management, new medications, and physical weakness, as they transition from hospital to home. They often have to return to the hospital shortly after being discharged, which can be burdensome for patients and their families. A video visit after hospital discharge may improve the post-discharge care and quality of life of recently hospitalized patients with cancer transitioning from the hospital to their home.

PRIMARY OBJECTIVES:
I. To assess the effect of CONTINUUM on patients’ confidence in managing their health condition, among patients recently hospitalized with advanced cancer.
II. To assess the effect of CONTINUUM on patient satisfaction with clinician communication, among patients recently hospitalized with advanced cancer.
III. To assess the effect of CONTINUUM on physical and psychological symptom burden, among patients recently hospitalized with advanced cancer.
EXPLORATORY OBJECTIVES:
I. To explore the effect of CONTINUUM on 30-day hospital readmissions in recently hospitalized patients with advanced cancer.
II. To assess potential moderators of the intervention effects on patient’s confidence in managing their health conditions and their satisfaction with their communication with their clinicians.
III. To explore whether change in patients’ confidence in managing their health condition as measured by the Patient Activation Measure-13 (PAM-13) is associated with 30-day hospital readmissions
OUTLINE: Patients are randomized to 1 of 2 groups.
GROUP I (USUAL CARE): Patients receive usual care and attend usual follow-up appointments. Patients also complete questionnaires over 10-15 minutes 10-15 days post-discharge.
GROUP II (INTERVENTION ARM): Patients receive CONTINUUM intervention consisting of attending a video visit over 30 minutes with nurse practitioner. Patients also complete questionnaires over 10-15 minutes 10-15 days post-discharge.
