Temporally-Modulated Pulsed Radiation Therapy versus Standard Radiation Therapy for the Treatment of Newly Diagnosed, IDH Wildtype, MGMT-Unmethylated Glioblastoma

This phase III trial compares a new type of radiation therapy, called temporally-modulated pulsed radiation therapy (TMPRT), with standard radiation therapy in treating patients with newly diagnosed, IDH-wildtype, MGMT-unmethylated glioblastoma. After completion of surgery, the usual treatment for glioblastoma includes radiation therapy (high energy x-rays) given together with a chemotherapy drug called temozolomide. About 70% of glioblastomas are MGMT-unmethylated, meaning they are less likely to respond to temozolomide chemotherapy. Because of this, radiation therapy plays a more important role in treating these tumors. Recent clinical trials testing new treatments for MGMT-unmethylated glioblastoma have not improved outcomes compared to the current standard of radiation therapy plus temozolomide. These studies also show that many patients – up to 80% – experience problems with memory and thinking after treatment. TMPRT is different from standard radiation therapy. Instead of giving the full dose of the daily treatment all at once, TMPRT delivers the same daily treatment in 10-13 “pulses,” with short 3-minute breaks between each pulse. Researchers are studying whether TMPRT, when combined with temozolomide, may help patients with glioblastoma live longer and better preserve memory and thinking compared to standard radiation therapy.
