This clinical trial evaluates multimodal community-level outreach and individual-level smartphone interventions for improving knowledge about, and uptake of, prostate cancer screening in African American men living in Oklahoma. Prostate cancer is the most diagnosed cancer in African American men and the second-leading cause of cancer-related death. The prostate-specific antigen (PSA) test is an early detection screening tool for prostate cancer, however, knowledge about prostate cancer and screening benefits and risks and uptake of PSA screening remains low among African American men. This clinical trial aims to engage African American men in education about and participation in prostate cancer PSA screening at the community level using a culturally tailored social media campaign and mobile health vans at community health events. This clinical trial also offers men who aren't ready to participate in PSA screening the opportunity to receive individual-level intervention using a tailored application (app) called Genius, which includes culturally-tailored, novel features to support at-home PSA screening. This multimodal intervention may increase prostate cancer knowledge and uptake of PSA screening among African American men in Oklahoma.

PRIMARY OBJECTIVES:
I. Conduct a multimodal community outreach intervention to improve awareness of and access to PSA screening.
II. Determine the efficacy of smartphone intervention on at-home PSA screening rates.
III. Explore the feasibility and acceptability of a social risk screening tool to connect participants to social needs referral services.
OUTLINE:
AIM 1 (COMMUNITY-LEVEL): Participants watch a culturally tailored educational video that covers topics related to prostate cancer and PSA screening and also includes a narrative-based testimonial from an African American (AA) prostate cancer survivor in Oklahoma over 10 minutes, either at home (if recruited through the social media campaign) or in the privacy of a mobile health van (if recruited at a community health event). Participants who are ready to screen after the educational video receive one-time navigation that includes a list of referral resources tailored to their social needs assessment and geography and may complete a PSA screening test in the privacy of the mobile health van (if recruited at a community health event), or may be directed to a webpage where they can order a free at-home PSA screening test kit (if recruited through the social media campaign). Participants who order a kit but do not return it within 7 days may receive text message reminders on days 8, 10, and 14. Participants who are not ready to screen after the educational video are offered participation in Aim 2. Participants who decline to participate in Aim 2 receive one-time navigation that includes a list of referral resources tailored to their social needs assessment and geography. Participants who agree to participate in Aim 2 proceed to Aim 2.
AIM 2 (INDIVIDUAL LEVEL): Participants are randomized to 1 of 2 arms.
ARM I: Participants receive access to the Task Force app, which includes generic health information about prostate cancer and screening, the ability to order an at-home PSA test, access to follow-up care for participants with abnormal PSA results, weekly check-ins with participant social needs navigators, and geofenced notifications of community organizations based on participant location and social needs, for 30 days on study.
ARM II: Participants receive access to the Genius app, which includes all the features of the Task Force app described above, as well as interactive learning modules about prostate cancer and screening, testimonial and educational videos, social support modules including experience sharing from AA prostate cancer survivors and the ability to talk with AA prostate cancer survivors, and an emphasis on intervention tailoring to participant learning progress, individual behaviors, culture, and health literary, for 30 days on study.
