This phase Ib trial tests the safety and side effects of DSP-0390 in combination with standard of care atezolizumab and evaluates how well the combination works in treating patients with small cell lung cancer that has spread outside of the lung in which it began or to other parts of the body (extensive stage) after receiving induction therapy. Maintenance therapy is treatment that is given to help keep tumors from coming back after it has disappeared following the initial therapy and may be given for a long period of time. DSP-0390 may stop the growth of tumor cells by blocking some of the enzymes needed for cell growth. Immunotherapy with monoclonal antibodies, such as atezolizumab, may help the body's immune system attack the tumor, and may interfere with the ability of tumor cells to grow and spread. Giving DSP-0390 in combination with standard of care atezolizumab may be safe, tolerable, and/or effective as maintenance treatment for patients with extensive stage small cell lung cancer (ES-SCLC).

PRIMARY OBJECTIVE:
I. To assess the safety of the emopamil binding protein (EBP) inhibitor DSP-0390 in combination with atezolizumab in patients with ES-SCLC in the maintenance phase.
SECONDARY OBJECTIVE:
I. To determine the preliminary efficacy of the EBP inhibitor DSP-0390 in combination with atezolizumab in patients with ES-SCLC in the maintenance phase.
EXPLORATORY OBJECTIVES:
I. To determine the additional preliminary efficacy of the EBP inhibitor DSP-0390 in combination with atezolizumab in patients with ES-SCLC in the maintenance phase.
II. To determine the impact of DSP-0390 on anti-tumor immunity (including myeloid-derived suppressor cells [MDSCs]) in ES-SCLC.
OUTLINE:
Patients receive DSP-0390 orally (PO) once daily (QD) on days 1-21 and standard of care atezolizumab intravenously (IV) on day 1 of each cycle. Cycles repeat every 21 days for up to 2 years in the absence of disease progression or unacceptable toxicity. Patients also undergo brain magnetic resonance imaging (MRI) during screening and blood sample collection and computed tomography (CT) throughout the study. Patients may also undergo additional brain MRI scans as clinically indicated throughout the study.
After completion of study treatment, patients are followed every month for up to 6 months.
