Feasibility and Acceptability of the Care Planning Framework (ABC123) for Advanced Cancer Care

This clinical trial tests the feasibility and acceptability of a care planning framework (ABC123) for patients with cancer that has spread to other places in the body (advanced). The ABC123 framework for older adults with advanced cancer is aimed at increasing the concordance between the treatment plan a patient would prefer and the treatment plan that is medically possible by combining existing tools that represent the principles of geriatrics (the study of health and care of old people), oncology (the study of cancer), and palliative care (treatment to relieve symptoms and reduce suffering). This trial may help doctors learn more about using a care planning framework (ABC123) to make sure that certain aspects of health and personhood are included in discussions around cancer and cancer treatment.

Inclusion Criteria

Provision to sign and date the consent form
Age > 65 years old, or > 60 with at least one deficit on a geriatric screen
Stated willingness to comply with all study procedures and be available for the duration of the study
Diagnosis of a solid-tumor cancer that is not curable with surgery or radiation within the 6 mos. prior to consenting. And that would potentially be treated with systemic cytotoxic chemotherapy. If molecular marker status is pending for a particular tumor, patients may still be approached for enrollment in the study.
New patient consultation within the University of Colorado (UC)Health Denver Metro system or newly diagnosed with recurrent metastatic/incurable disease
English speaking only for the pilot portion
CLINICAL STAKEHOLDERS: Must be a member of the staff and provider team within a clinical pilot site or a cancer center leader

Exclusion Criteria

Have a diagnosis of a different advanced cancer that has required systemic therapy
Have any condition that, in the opinion of the investigator, would compromise the well-being of the subject or the study or prevent the subject from meeting or performing study requirements

PRIMARY OBJECTIVES:

I. Assess the preliminary feasibility and acceptability of ABC123 through a single-arm pilot study.

II. Evaluate ABC123 as measured by goal concordant care.

OUTLINE:

Patients complete physical function testing at session 1. Beginning 0-7 days later, patients then participate in guided goals of care discussion with an advanced practice provider at session 2.
