AGEN1571, Botensilimab, Balstilimab, and Liposomal Doxorubicin in Combination with Ultrasound-Mediated Blood Brain Barrier Opening for the Treatment of Patients with Newly Diagnosed Glioblastoma
This phase IIa trial tests how well AGEN1571, botensilimab, balstilimab, and liposomal doxorubicin work in combination with ultrasound-mediated blood brain barrier opening, using the SonoCloud-9 device, for the treatment of patients with newly diagnosed glioblastoma. The SonoCloud-9 device is surgically placed in the skull and works by temporarily increasing the permeability of the blood brain barrier, using ultrasound and a medication called Definity, to allow increased uptake of the treatment medications to the tumor cells. AGEN1571, botensilimab and balstilimab are monoclonal antibodies that may interfere with the ability of tumor cells to grow and spread. Doxorubicin is in a class of medications called anthracyclines. Doxorubicin damages the cell’s DNA and may kill cancer cells. It also blocks a certain enzyme needed for cell division and DNA repair. Liposomal doxorubicin is a form of the anticancer drug doxorubicin that is contained inside very tiny, fat-like particles. Liposomal doxorubicin may have fewer side effects and work better than other forms of the drug. It also may enhance the reaction to the immunotherapy medications. Giving AGEN1571, botensilimab, balstilimab, and liposomal doxorubicin in combination with ultrasound mediated blood brain barrier opening may kill more tumor cells in patients with newly diagnosed glioblastoma.

PRIMARY OBJECTIVE:
I. To determine the safety, and feasibility of the SonoCloud-9 device (SC9) implantation and repeat sonication with the device while injecting microbubbles (SC9/MB) to temporarily open the blood-brain barrier (BBB), in combination with intravenous administration of dual checkpoint inhibition with the state of the art second generation agents botensilimab (BOT) and balstilimab (BAL), +/- anti-ILT2 monoclonal antibody AGEN1571 (AGEN1571), and low-dose liposomal doxorubicin (DOX) which is given as an immunomodulator.
SECONDARY OBJECTIVES:
I. To determine whether our cohorts exhibit favorable overall survival (OS) compared to historical controls.
I. To investigate the correlation between biomarkers (phosphorylated [p]ERK and major histocompatibility complex [MHC] class 2 expression in macrophages) and outcome in the patient cohorts.
EXPLORATORY OBJECTIVES:
I. To evaluate the association of other putative biomarkers of glioblastoma (GBM) susceptibility to immune checkpoint blockade (ICB).
II. To determine whether the proposed therapy induces on T-cell responses to tumor/patient-specific neoantigens, and whether these correlate with OS.
III. To investigate the patterns of recurrence by magnetic resonance imaging (MRI) in patients undergoing ultrasound mediated blood brain barrier opening.
IV. To investigate whether nucleic acid/exosomes and peripheral blood cells (from blood samples collected before and after every sonication) are correlated with outcomes.
V. To evaluate the effect of therapy on tumor tissue before, during, and after the treatment.
VI. To evaluate the effect of ultrasound-based BBB opening on the brain connectivity and neurological function by functional (f)MRI.
VII. To evaluate and describe outcomes between cohorts of patients treated with BOT/BAL/sonication/DOX alone and with the addition of AGEN1571.
OUTLINE: Patients are assigned to 1 of 2 cohorts.
NEOADJUVANT COHORT: Patients undergo surgery to place the SC9 device. Beginning 7 to 15 days after surgery, patients receive AGEN1571 intravenously (IV) over 30 minutes on day 1 of each cycle, balstilimab IV over 30 minutes on day 1 of each cycle, and botensilimab IV over 30 minutes on day 1 of cycles 1, 3, 5, and 7. At the end of the infusion(s) on day 1 of each cycle, patients receive perflutren lipid microspheres (Definity) IV over 20 seconds and the SC9 device is accessed transdermally over 4-5 minutes to activate the device. Patients then receive liposomal doxorubicin IV over 30 minutes on day 1 of each cycle. Beginning 2-3 weeks after start of cycle 1, patients undergo surgical resection. Beginning 2-4 weeks after completion of standard of care temozolomide and radiation therapy, patients resume cycle 2. Cycles repeat every 21 days for up to 8 cycles, in the absence of disease progression or unacceptable toxicity. Treatment may continue for up to 2 years in the absence of disease progression if the treating oncologist deems it in the patient’s best interest.

At the end of the infusion(s) on day 1 of each cycle, patients receive Definity IV over 20 seconds and the SC9 device is accessed transdermally over 4-5 minutes to activate the device. Patients then receive liposomal doxorubicin IV over 30 minutes on day 1 of each cycle. Cycles repeat every 21 days for up to 8 cycles, in the absence of disease progression or unacceptable toxicity.
