This clinical trial studies whether a survivorship care planning program, Patient-owned Survivorship Transition Care for Activated, Empowered survivors-Ovarian (POSTcare-O), supports ovarian cancer survivors in transitioning to survivorship care. The experience of cancer survivors transitioning from active treatment to survivorship is marked by uncertainty. Patients often experience anxiety and uncertainty about follow-up care and have a desire for more information about treatment and late effects of treatment. The POSTcare-O program involves a single coaching encounter with a trained nurse navigator using communication strategies to engage survivors and help with the creation of patient-owned survivorship care plans. The survivorship care plans incorporate health goals and strategies related to surveillance, symptom management, and health behavior. Patients also receive support to engage with the survivorship care plan through follow up phone calls with the trained nurse navigator. Participating in POSTcare-O may help support ovarian cancer survivors in transitioning to survivorship care, which may lower anxiety and uncertainty.

 PRIMARY OBJECTIVES:
I. Test the efficacy of the POSTCare platform in supporting QOL, reducing depressive symptom burden, and reducing recurrence worry.
II. Examine factors associated with the impact of the intervention.
III. Understanding aspects of care platform design and delivery that are likely to affect the potential for dissemination.
OUTLINE: Patients are randomized to 1 of 2 arms.
ARM I: Patients attend a POSTcare-O survivorship education session over 30-60 minutes and receive a POSTcare-O survivorship plan on study. Patients also receive monthly nurse navigator phone booster follow-ups for 2 months as well as an optional follow-up before patients' follow-up visit at 3 months.
ARM II: Patients receive standard survivorship education on study.
After completion of study intervention, patients are followed up at 3 and 6 months.
