Patient Navigation for the Improvement of Outcomes and Quality of Life in Medically and Socially Vulnerable Cancer Patients, PARTNERS Trial

This clinical trial studies whether a patient navigation program (PARTNERS) improves outcomes and quality of life in medically and socially vulnerable cancer patients. Medically and socially vulnerable patients suffer disparities regarding cancer screening, diagnosis, and treatment. One reason is the disconnected nature of the healthcare system. Communication across institutions/organizations can enhance the ability to address social barriers that may interfere with timely access to care. The PARTNERS program gives patients access to a patient navigator who serves as a resource for the patient and helps the patient navigate and get connected to local resources. The patient navigator can also access a platform (CONNECT) which provides access to patient information across institutions in the region to aide in providing a more holistic approach to addressing patient needs. This may address patients' needs sooner and may improve access to care. Participating in the PARTNERS program may improve outcomes and quality of life in medically and socially vulnerable cancer patients.
