  • Risk Enabled Therapy after Neoadjuvant Immunochemotherapy for the Treatment of Bladder Cancer

    This phase II trial studies the effect of nivolumab when given together with chemotherapy drugs in treating patients with muscle invasive bladder cancer. Immunotherapy with monoclonal antibodies, such as nivolumab, may help the body's immune system attack the cancer, and may interfere with the ability of tumor cells to grow and spread. Chemotherapy drugs, such as the accelerated methotrexate / vinblastine / adriamycin / cisplatin (AMVAC) regimen, work in different ways to stop the growth of cancer cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Nivolumab in combination with the AMVAC chemotherapy regimen may help patients avoid or delay surgery and improve the quality of life and long term survival of patients with bladder cancer.

  • Testing the Addition of MEDI4736 (Durvalumab) to Chemotherapy before Surgery for Patients with High-Grade Upper Urinary Tract Cancer

    This phase III trial compares the effect of adding durvalumab to chemotherapy versus chemotherapy alone before surgery in treating patients with upper urinary tract cancer. Immunotherapy with monoclonal antibodies, such as durvalumab, may help the body’s immune system attack the cancer, and may interfere with the ability of tumor cells to grow and spread. Chemotherapy drugs, such as methotrexate, vinblastine, doxorubicin, cisplatin, and gemcitabine work in different ways to stop the growth of tumor cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Durvalumab in combination with chemotherapy before surgery may enhance the shrinking of the tumor compared to chemotherapy alone.

  • Accelerated or Standard BEP Chemotherapy in Treating Patients with Intermediate or Poor-Risk Metastatic Germ Cell Tumors

    This randomized phase III trial studies how well an accelerated schedule of bleomycin sulfate, etoposide phosphate, and cisplatin (BEP) chemotherapy works compared to the standard schedule of BEP chemotherapy in treating patients with intermediate or poor-risk germ cell tumors that have spread to other places in the body (metastatic). Drugs used in chemotherapy, such as bleomycin sulfate, etoposide phosphate, and cisplatin, work in different ways to stop the growth of tumor cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Giving BEP chemotherapy on a faster, or “accelerated” schedule may work better with fewer side effects in treating patients with intermediate or poor-risk metastatic germ cell tumors compared to the standard schedule.

  • Trastuzumab, Pertuzumab, and Combination Chemotherapy in Treating Patients with Locally Advanced, Resectable, or Inflammatory Her2 Positive Breast Cancer

    This phase II trial studies how well two different anti-cancer treatment regimens which both contain trastuzumab and pertuzumab, but with different combinations of chemotherapy work in shrinking cancer before surgery in patients with localized Her2 positive breast cancer. Trastuzumab and pertuzumab are forms of “targeted therapy” because they work by attaching to specific molecules (receptors) on the surface of cancer cells, known as HER2 receptors. When trastuzumab or pertuzumab attaches to HER2 receptors, the signals that tell the cells to grow are blocked and the cancer cell may be marked for destruction by the body’s immune system. Drugs used in chemotherapy, such as doxorubicin, cyclophosphamide, paclitaxel, docetaxel, and carboplatin, work in different ways to stop the growth of tumor cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Giving trastuzumab with pertuzumab, in addition to combination chemotherapy has been shown to be very effective in shrinking cancer before surgery in patients with Her2 positive locally advanced breast cancer. This trial aims to help determine which regimen may work best with the least toxicity.
